I think that the best friend referred to in this song is Stevie's late friend, Robin, and that the song captures how she was feeling when she knew her friend had a fatal case of leukemia and took her to Hawaii to cheer her up. She doesn't know what she can say or how she should act in the situation, how to comfort her and make her feel better about something as horrible as dying. Stevie says that she can deal with Robin's mother better, help her, but she doesn't know how to help Robin. After all these years that they've known each other, it ends like this: her friend had to run away. Then, Stevie wants to find Robin, to see her, but she can't, because her friend has died. A very sad song.
